Thank you for choosing Patchwork Design Inc., alias "Patchworks". We specialize in custom patches, patch programs, lanyards, t-shirts, pins etc.

We also strive to provide complete customer satisfaction. This sales agreement will assist you in making a decision on the actual number of products that you need to purchase for your company or organization. It provides you with an outline of pricing, deposits and additional charges. This agreement is not your final bill and is only used as a signed agreement of this purchase. We value you as a customer and look forward to a lasting business relationship.

 Please note that several different colors on a design accrues additional fees to each shirt. Each design is considered a separate production run. (This is included in the pricing)

We offer up to 6 colors on a t-shirt.   When choosing your color(s), please be careful in the color of t-shirts that are chosen. For example, if you are doing an event with a one color black screen print, you wouldn't want anyone to choose a dark colored t-shirt as their teams color. Minimum order for a 3 color shirt is 36 shirts.

Pricing per the complete order:     For one color shirts use chart on T-shirt page. For multi color selections or large orders, please contact us for pricing and shipping.

2X and 3X t-shirts are an additional $2.50 a shirt.

If you have trouble viewing the color codes or color names. Please contact us and we will assist you in your choices.

As long as you use the same design, you may have it printed on many different items of your choice. Example: T-shirt, totebags, polo, sweat shirt, sweatpants etc. Each product counts under quantity which gives you more reasonable pricing.

Due upon completion of this order, is 50% of the total amount of the purchase order. This amount will be deducted from your invoice. After completion of this agreement an invoice and shipping date will be mailed. The remainder of the fee will be due on the date specified on your invoice. Usually 5 days prior to shipping.

Net 30 accounts are available on large purchases.

When using credit card, please the credit card information at the bottom of this form.

Returns: Patchworks offers no returns due to it being a customized item specifically designed for your company or organization.

Delivery of your order: Most items are shipped US mail or UPS. Re-orders of a customized product are delivered within 3 to 4 weeks. Please note when reordering your item you are paying the price per item, according to the pricing amount of your second order. For example, if your first order of t-shirts was 500, but your second order was 12. The pricing of the first order will be for 500 t-shirts and the second order will be for 12 shirts.
